Health Promotion:

Staying fit and healthy is an important part of maintaining good mental and physical fitness at any age. If a customer has an interest and ability to participate in local -activities, our care workers are able to support them and be involved as part of the homecare plan. Or perhaps a customer may want support to do some simple activities at home.

Engaging in a healthy active lifestyle helps stimulate the brain and assists in staying independent.  It can also be immensely enjoyable.

Assessments and Referrals:

We design admission prevention schemes around all conditions for our service users. Close monitoring of a service users health coupled with close liaison with local based community services enables us to obtain a quick response should the service user require the assistance of the Occupational Therapist, GP, Physiotherapist, District Nurse or CPN.

The primary aim of this service is to reduce the need for hospital visits and admission as well to ensure that that the service users lead as comfortable, without fear and independent life in their community . So when you are looking at one to one care it is our expertise and experience that will notice detailed changes in our service user’s health. Quick Response times mean that prevention of hospital admission is often eliminated.

We continuously risk assess our service users and have various screening tools to help reduce the risk of dehydration, falls and pressure sores to name a few.

Sleeping Night:

With our sleep-in night service one of our Care Workers will sleep in your home during the night hours and provide you with peace of mind.

Some of the specific benefits of physical activity are:

  • Reducing the risk of developing chronic diseases;
  • Aids in the management of active problems such as high blood pressure, diabetes, obesity and high cholesterol:
  • Improves the ability to function and stay independent in cases where people have lung disease or arthritis;
  • Reduces symptoms of depression and pain; and
  • Improves balance and prevents falls.

We encourage and support our service users to take part in simple, healthy activities, which may include something as basic as walking. Of course, participation in any activity will depend on the service user’s choice and abilities. A member of our team will discuss your preferences with you as part of your tailored care and support plan.

Our dedicated team of home care workers are fully trained to:

  • Assist with a ‘good morning’ start to the day, helping you to get up, wash, shower or bathe, get dressed and have breakfast
  • Remind or assist you to take your medicines and collect or return medication from your pharmacy or dispensing GP surgery
  • Prepare meals with, and assist you at mealtimes where required.
  • Shop with you or help you to make a shopping list, go to the shops, come back and assist you with putting it all away
  • Assist you with your  laundry or ironing  and assist you with keeping your home clean and tidy
  • Support you with social activities such as going out for a walk, attending a day centre, visiting friends or family, going to your church or club etc.
  • At the end of each day, offer some help or assist with getting ready for bed.
